Out-of-Network Therapy: How Reimbursement Actually Works in New Jersey

Many of the most experienced therapists in Bergen County do not participate in insurance networks. That does not mean insurance cannot help pay for your care — it means the payment works differently. Understanding out-of-network benefits often changes the math on private therapy more than families expect.

What Does Out-of-Network Mean?

An out-of-network provider has no contract with your insurer. You pay the practice directly at the time of service, and then — if your plan includes out-of-network benefits — your insurer reimburses you a percentage of the cost. PPO plans typically include these benefits; HMO plans typically do not — but exceptions run in both directions. The single most important step is confirming what your specific plan covers.

What Is a Superbill?

A superbill is an itemized receipt the practice provides, containing everything your insurer needs to process reimbursement: provider credentials, diagnosis code, service codes, dates, and fees. You submit it to your insurer through their member portal or app. Many families also use services that automate the submission. Reimbursement typically arrives as a check or deposit within a few weeks.

What Will You Actually Get Back?

Three numbers determine it. Your out-of-network deductible: what you pay before reimbursement begins. Your coinsurance rate: the percentage the plan covers afterward, which varies widely by plan. And the plan's "allowed amount": the fee the insurer considers reasonable for the service, which reimbursement is calculated from — sometimes lower than the actual fee. Plans differ enough — including EPO and POS plans, and PPOs that exclude out-of-network mental health — that the only reliable answer comes from calling the member services number on your card and asking those three questions specifically for outpatient mental health.

Why Do Many Experienced Therapists Stay Out of Network?

In-network reimbursement rates often force high session volume, which affects the depth and flexibility of care. Out-of-network practice allows longer sessions where needed, unhurried scheduling, and treatment decisions driven by clinical judgment rather than insurer authorization. Families weighing the cost difference are usually weighing exactly this trade.
